Sever’s disease is one of the most common causes of heel pain in children and adolescents, particularly those who are active in running and jumping sports. Despite its name, it’s not a “disease” but rather a temporary condition related to growth. At Seaforth Physio, we frequently treat young athletes experiencing this issue and help them return to sport safely.
Sever’s disease, or calcaneal apophysitis, occurs when the growth plate in the heel becomes irritated due to repetitive stress. It usually appears in children aged 8–14 during periods of rapid growth, when bones grow faster than muscles and tendons can adapt. Sports like football, soccer, basketball, netball, and gymnastics often bring on symptoms because of the repeated impact through the heels.
Heel pain during or after physical activity
Limping or walking on tiptoes to avoid pressure on the heel
Tenderness when squeezing the back or sides of the heel
Stiffness in the calf or Achilles tendon
Symptoms often ease with rest but return once activity resumes, making it frustrating for both kids and parents.
1. Pain Management and Education
The first step is reducing pain and inflammation. At Seaforth Physio, we guide families on activity modification—adjusting training loads, using ice, and sometimes recommending heel cushions or supportive footwear.
2. Stretching and Mobility
Tight calf muscles are a major contributor. We prescribe gentle stretches to improve flexibility in the calf and Achilles, reducing stress on the heel.
3. Strengthening Exercises
Strengthening the lower leg and foot muscles improves shock absorption and control, helping prevent flare-ups when kids return to sport.
4. Biomechanical Assessment
Our physiotherapists assess foot posture, running style, and growth patterns. In some cases, taping or orthotic advice may help reduce strain on the heel.
5. Gradual Return to Sport
We work closely with young athletes to ensure they return to training and competition safely. A progressive program helps them regain confidence while avoiding setbacks.
